This PDF can be printed/filled out by hand or you can type the information directly on the form with the Adobe Fill and Sign feature in Adobe Reader. First open the file above. Then download it with the File Icon containing the Down Arrow at the top right of your browser. With the file on your computer, open the file on your computer using Adobe Reader.
